Dr. Josephs practice was placed under probation for 18 months beginning in April, 1993 and ending on 10/31/1994. Within 6 months Dr. Josephs will submit copies of all charts of patients undergoing bio-oxidative therapy subsequent to an informal interview for Board review. He must complete pre-approved course in practice risk management within one year of the date of the commencement of probation. Dr. Josephs must undergo a chart review within 6 months of the date of the probation. Within 18 months of the onset of probation Dr. Josephs must present evidence to the Board of satisfactory completion of a general medical competency examination.

Dr. Josephs successfully complied with the Order as required by the terms and conditions.
