On July 3, 1997 Dr. Olsztyn's license 004 was placed on probation for a period of three years. In addition the probation included a period of 60 day suspension from practice. Dr. Olszyn shall pay a civil penalty of $1000 and reimburse the Board for all consts incurred in the matter. Dr. Olsztyn shall retain and pay for a professional practice monitor who shall develop a plan for corrective action and monitor that plan. He shall continue in psychotherapy and submit quarterly status reports to the Board on his progress in the treatment program. He shall submit to and pay for any random biological fluid testing for not less than four times per year. The practice monitor shall monitor all prescriptions, both written and called in according to a written plan agreed to by the parties and attached to the Consent Agreement as Addendum 1. During his probation Dr. Olsztyn shall obtain 12 hours of continuing medical education in medical ethics and 12 hours of continuing education in pain management.

This Consent Agreement was terminated for successful completion of all of the terms on July 24, 2000.
